Important Information Before Registering for a Personal Health Record (PHR) Account
Our online booking system has changed, to make it easier for you to manage your own appointments and details.
Before registering for a Personal Health Record (PHR) account, please read all the information below.
Please note: We have a range of appointment types available, which we open in advance. This can be as little as 48 hours in advance, or up to 6 weeks in advance. DISCLAIMER
If this is your first time using PHR, you will need to register for a new account. You must only register for YOURSELF with YOUR OWN personal email address and YOUR OWN UK mobile phone number. Do not use shared email addresses and make sure no one else has access to your phone. By registering for a PHR, you are giving your consent for us to contact you using the email address and mobile number provided. We recommend password-protecting your phone and email accounts with strong passwords.
By registering, you confirm you are providing us with accurate, up-to-date information and the email address and mobile number you provide is secure and only accessible to yourself. Please take care to ensure the information is correct as it cannot be changed once submitted.
PHR accounts that are in breach of these terms, by using someone else’s email address or phone number, will be deactivated pending identification verification to change the details. There may be occasion when we will ask you to verify your identity.
Personal Health Record (PHR)
Your Personal Health Record (PHR) provides you with the ability to:
- Register with the OpenClinic service.
- Manage your own details.
- Book, cancel and re-arrange your appointments.
The following applies to online appointment booking:
- Only 1 appointment can be made online. If you need any additional appointments, please call 0808 178 0955 (if you live in Stoke, Staffordshire and Telford & Wrekin) or 0300 404 2996 (if you live in Shropshire).
- The PHR will ask a series of questions to establish your care needs, which allows us to offer you the appropriate service. If an appointment is offered, this may be via telephone or a face-to-face appointment at your chosen clinic.
Important notes for appointments booked online via PHR:
- Please arrive 10 minutes BEFORE your appointment time. If you have not arrived by your designated appointment time, your appointment may have to be re-booked.
- This is especially important for appointments to insert or remove a contraception method involving a minor procedure (e.g., the implant, hormonal coil or copper coil). This allows time for necessary investigations, including a urine sample and/or blood pressure check. Should you not arrive in time to do this, we may not be able to complete your procedure during your scheduled appointment.
- If you want to change your contraception method (for example, to remove an IUD/IUS and replace with an implant, or, to remove an implant and replace with an IUD/IUS), please phone us on 0808 178 0955 (if you live in Stoke, Staffordshire and Telford & Wrekin) or 0300 404 2996 (if you live in Shropshire) before making an appointment online to discuss your needs. It might be necessary to book a double appointment, which can only be done over the phone. DO NOT TRY TO BOOK THIS ON PHR.
- Please be aware that due to the nature of our services, it is not always appropriate for children to be present during your consultation. If you bring children with you, we may not be able to complete your consultation and a further visit may be necessary.
- We have male and female sexual health staff working in our clinic, who carry out intimate examinations and coil procedures. We cannot guarantee whether your clinician will be male or female on the day.
REGISTRATION PROCESS
Once you have submitted your details, a verification email will be sent to confirm you have access to this email account. Once you have verified the email by clicking on the link sent to your email address, you will then be sent a verification code via text message to your mobile phone number. Once you have entered this code, you will be able to register for your PHR account. Please enter the verification code carefully as you only have 3 attempts.
Before proceeding to register for a PHR online booking account, it is important that you read the information below.
If you need access to any of the services listed below, you do not need to register for a PHR online booking account, as most of these services can be accessed online without an appointment.
Online STI Testing
If you have do not have any symptoms and would like a test for sexually transmitted infections (STIs), you can order a free test by clicking the relevant link below:

Emergency Hormonal Contraception (Morning After Pill)
If you have had unprotected sex and don’t want to get pregnant, you can take the Emergency Hormonal Contraceptive (EHC) pill, also known as the morning after pill. This can be taken up to 120 hours or five days after unprotected sex. EHC is more effective the sooner you take it, so act fast. We advise calling the pharmacy in advance to ensure it is open, has emergency contraception in stock and the staff to provide it.
PEP
If you have had sex with someone without a condom in the last 72 hours (3 days), who has HIV or thinks they may have HIV, please DO NOT use the online booking system.

If it is out of hours, please attend your local A&E as soon as possible. There is a treatment called PEP (Post Exposure Prophylaxis) that may reduce your chances of getting HIV if started within 3 days (72 hours) of being exposed.

C-Cards
We do not offer appointments online for condoms or C-Cards. You can pick up condoms using your C-Card from a range of venues across Stoke-on-Trent, Staffordshire, Shropshire and Telford & Wrekin. Frequently Asked Questions
How do I register for a PHR account?
To register for a PHR account, you need to accept the 3 terms and conditions boxes above, then select ‘Register Now’. You will then be directed to the PHR login page, where below it will give you the option to register for a PHR account.
You will need to provide a private email account and mobile number and verify both as part of the registration process. You will need to verify your email address within 24 hours, if you do not verify within this time period you will need to start the registration process again.
If you haven’t received a verification email when signing up to the PHR check your junk folder. If no email has been found, click on the ‘Help’ button on the registration page and select ‘Resend registration confirmation email’.
Once you have verified your details by following the link sent via email, you can log in with your email and password.
Once you have logged in, you will be taken to the verification code screen where you will need to input the code sent to the mobile phone number you registered with and select verify. You will have three attempts to enter your verification code. If you do not receive the code, you can use the ‘Resend’ button.
You will then be taken to the ‘My Details’ section of your PHR account, where you will need to enter your details.
What do I do if my postcode isn't recognised?
We use a national database of postcodes to find addresses, which is not always up to date. If your postcode is not recognised, please call us on 0808 178 0955 (residents of Stoke, Staffordshire and Telford & Wrekin) or on 0300 404 2996 (residents of Shropshire) and we will send you a direct invite to register for your PHR account.
What information will I need to provide?
To sign up for a PHR account, you will need to provide your email address, mobile phone number, home address and some additional personal information including full name, date of birth, gender identity and sex assigned at birth.
How do I login to my PHR account?
If you have already created a new PHR account, you can login to your account to book and manage appointments via the ‘Register Now’ button at the bottom of the online booking page. You will need the same details and password you created upon your registration.
How do I update my email and/or phone number in my PHR account?
For security reasons, you are unable to change your email and mobile number on your PHR account. Should you wish to make a change, please contact us on 0808 178 0955 (residents of Stoke, Staffordshire and Telford & Wrekin) or on 0300 404 2996 (residents of Shropshire). You will need to verify your identity before we can make these changes.
What do I do if I've forgotten my password?
You can reset your password on the login page by clicking the ‘Help’ button. By doing this, you can choose to either reset your password or re-send your registration confirmation email.
How do I book and manage my appointments?
Once you have registered and logged in to your PHR account, you can book an appointment by going to the ‘Appointments’ section on the homepage and clicking the ‘Book Appointment’ button.
You will be asked a series of questions to find the right appointment type for you. Based on your answers, you may be offered a telephone or face-to-face appointment, or be signposted to the correct services. If you require an appointment to change your contraception method, please phone us on 0808 178 0955 (residents of Stoke, Staffordshire and Telford & Wrekin) or on 0300 404 2996 (residents of Shropshire) to discuss your needs before booking online. It might be necessary to book a double appointment. If this is the case, we will book this for you over the phone. DO NOT TRY TO BOOK THIS THROUGH PHR.
You can also view and manage your appointments from the ‘Appointment’ section. To cancel the appointment, click the ‘Cancel’ button next to your appointment information. If you then need to re-arrange, select the ‘Book Appointment’ button.
Will I receive a confirmation of my appointment?
Once you have booked your appointment, you will be able to view and manage the appointment in the ‘Appointments’ section of your PHR account. You will receive an appointment reminder by text message 48 hours before your appointment.
I don't have Internet access. Can I still register for a PHR account?
If you are unable to access your PHR account for any reason, please call us on 0808 178 0955 (residents of Stoke, Staffordshire and Telford & Wrekin) or on 0300 404 2996 (residents of Shropshire) during clinic opening hours.
I can't find my GP details. What do I do?
Some users may experience issues when entering their GP details. If you cannot find your GP, please tick the option which states ‘Continue without GP details’.
